How the course ran
Rolling and boxcar scarring across both cheeks, three years after his acne had been controlled with a course of isotretinoin — well past the six-month waiting period before energy-based treatment.
We assessed under angled light and confirmed a mixture of rolling scars, which respond well to deep remodelling, and a smaller number of ice-pick scars, which do not. He was told at the outset that the ice-pick scars would need a different approach and would not improve with this course.
Four radiofrequency microneedling sessions, five weeks apart, at conservative energy settings for his skin tone, with a full seven days of bland aftercare after each.
The photograph is at six months, which is when collagen remodelling has largely finished. The improvement is a clear softening of the rolling component rather than erasure. He is happy with it, and he knows what it is and is not.
